The Dream Items (Shirt, Bag, Umbrella) are the things that are on Kitty Shop right now. You can click on its tab for more information about it, but basically, these are concept items that Sanrio Town may be releasing to the public which are designed by the users themselves.

The Kitty Shop is really a shop in the truest sense, which means that it does sell items.
Currently, the Concept/Dream Items are on Phase 2, so do wait for Phase 3, when the items will already be up for sale.

Actually, no. For starters, you design the items first, so that option comes prior to the buying.:P Actually purchasing the item, however, is not required to choose which design you want.
I suggest that you visit the Kitty Shop so it will be clearer to you.:)
